There are 4 passport size photographs required, 1 attached with the form and the other 3 safely enclosed in an envelope with self signature on the backside. Also the attested copies of certificates of educational qualification, age proof, caste certificate and permanent address proof are required to be enclosed with the application form. The self addressed reply envelope affixed with postage stamp of Rs. 5/- received at P.H.Q Itanagar and challan to the given address, PHQ, Itanagar on or before 30-09-2013.
